Our office policies are presented here for your information and understanding.
  1. Patient Information: All patients must complete our patient information form prior to seeing the doctor. 

  2. Financial Policy: After the doctor does her evaluation, one of our financial coordinators will discuss professional fees with you as well as your financial arrangements. Payment in full is required at the time of the service.

    1. We accept cash, checks, Visa, MasterCard, Discover and American Express.

    2. Long term financing with low monthly payments and interest-free options for up to 12 months are available with CareCredit or First Financial application and acceptance.

    3. Any account not paid in 60 days will be turned over to an outside collection agency and late fees may apply. 

    4. Every effort is made by our staff to give estimates for all treatment prior to the occurrence. We would like you to keep in mind that costs could change according to necessary procedures that may occur during performance of the treatment.

  3. Regarding Insurance: Our office understands that you have dental insurance. This is a benefit provided for you by your employer. It is your responsibility to know your insurance plan. Our treatment planning is based on your needs as a patient and not according to your insurance coverage. We are happy to try and help answer questions about your dentistry and your insurance claims, but you are ultimately responsible for your treatment and the associated costs involved.

  4. Minors must be accompanied by an adult: Any patient under 18 years of age must have consent from a legal guardian before treatment can be started. The adult accompanying a minor or his or her guardian is responsible for full payment at the time of service.

  5. Missed appointments: Our office requires notice of cancellation on surgery appointments 48 hours prior to the surgery. When your appointment is made, we have set aside that time exclusively for you. When a patient does not show up for an appointment or cancels at the last moment, other patients are deprived of the treatment time. Cancellations with less than 48 hours’ notice may result in a fee for a percentage of that service

  6. Transfer of Records: There will be a $25.00 fee for duplicating radiographs and records for transfer to another provider. Texas State Law requires we keep all originals in our office for no less than 5 years.
